The mission of Coastal Community Health Services, Inc. is to embrace our community by seeking to meet the primary and preventive health care needs of the medically underserved residents in diverse settings with quality, affordable, and accessible health care and supportive services. We will help our community to become healthy by providing access to care regardless of ability to pay, race, religion, or other barriers. We will support the development of empowered local leadership that values a healthy community.
In November 2013, Coastal Community Health Services (CCHS), received grant funding and was designated by the U.S. Department of Health and Human Services, under Section 330 of the Public Health Service Act (PHS), as a Federally Qualified Health Center*. Also known as Community Health Centers, they must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors.
Patient-Centered Medical Home
PCMH is not a building, a house, or a hospital. It is a Model of Care (MOC) designed to improve the coordination of your health care with an emphasis on your all-around well-being. While we believe we have been providing patient-centered care, encouraging a partnership with each of our patients, PCMH goes further. Learn more about the new way we will manage your health care.
We Promise to Serve All Patients
We offer discounted fees for patients who qualify.
We will not deny services based on a person's race, color, sex, national origin, disability, religion, sexual orientation, inability to pay
